Practice Closure

It is with mixed emotions that we announce the closing of Sierra Pulmonary & Sleep Institute.
Our office will permanently close on Thursday, November 16, 2017.
Due to forces beyond our control, including Medicare requirements, hospital control of the local referrals and challenges with private insurance reimbrusement, we have decided to cease operations and refer our patients to other local and regional providers for their continued care (see link below). 
We have greatly appreciated the priveledge and opportunity to care for you over the years and hope you find the care you deserve in our absence.

Before making a formal record request, please check any paperwork that was given to you at your office visits. It was our practice, at every visit, to give patient records from the previous office visit and/or testing visit.
We will be processing all record requests, once a week, on Friday afternoons. Please print and fill out the Medical Record Release form highlighted below. You can scan and email or mail to P.O. Box 50254, Sparks NV 89435. Once requests are received, you will be contacted by phone or email regarding the charge which can be paid by check/money order.  We are only providing records to patients, not providers. 

There will be a .60 cent per page plus postage charge for medical records. 


Below is a comprehensive list of local and regional providers in pulmonary, sleep and allergy care, as well as recommended DME support for CPAP users if they choose to pay cash for their supplies. Please contact one of these providers as soon as possible to establish care.
